If you have not been previously assisted, you must use our initial request form. As a reminder, in order to be eligible to receive assistance from SOAR, you must meet the following criteria. If you do not meet this criteria, then your form cannot be submitted.
- Current resident of Frederick County, Maryland
- At least 65 years old
- Income does not exceed $2,100/mo for individuals or $3,000 per couple.
